Inform Court Form Update - Ontario Labour

An update to the Ontario Labour form suite was released today for Inform Court Form Suite subscribers.

Updated Forms:

1. Request for Appointment of Single Arbitrator Under Section 49
2. Request for Appointment of Arbitrator/Chair/Nominee Under Section 48
   

The above forms were updated to reflect the recent address change of the Office of Arbitration. The office is now located at 400 University Avenue, 8th Floor, Toronto, Ontario, M7A 1T7. Please note that the Office of Mediation and the Collective Agreements Library have also moved to the 8th floor of 400 University Avenue. The Employment Standards Practices Branch remains on the 9th floor.
